Immunizations

General Immunization Information

 To help keep you and your family healthy, ELVPHD provides many vaccines for children, adults and teens.   The majority of vaccines are covered by insurance as preventative medicine without requirement of a copay or co-insurance to meet your deductible. However, all people interested in receiving vaccines should check the immunization coverage allowed with their specific insurance plan.

We serve clients with private insurance, Medicare and Medicaid. Following is a list of major insurance companies currently accepted:

  • United HealthCare
  • Coventry
  • Medicare

  • Medicaid (Total care, Well Care, United HealthCare)

We want to make getting immunizations as easy as possible because we know how busy life is!  We have three locations to serve you (we just ask that you make an appointment ahead of time so that we can ensure a nurse is on site) and we host immunization clinics in several towns in our service area. Also, we offer businesses the convenience of coming to the job site to provide flu shots and other optional vaccines. Private Immunizations (Children and Adults)

To help keep you and your family healthy, ELVPHD provides all Centers for Disease Control and Prevention (CDC) recommended vaccines for children and teens. The majority of vaccines ELVPHD provides are covered by insurance as preventative medicine without requirement of a copay or co-insurance to meet your deductible. However, all people interested in receiving vaccines should check the immunization coverage allowed with their specific insurance plan. 

**Appointments are required for individuals with private insurance to ensure that we have the vaccine your child needs readily available.   To make an appointment please call 402-529-2233.

Child and Adolescent Vaccinations offered include:

Varicella (Chickenpox)

There must be a parent or guardian accompanying the child to receive immunizations.  A current copy of the child’s immunization records and health insurance card is needed.   If someone else brings the child to clinic they must have a completed proxy form and health questionnaire filled out and signed by the parent.

Vaccines for Children (VFC) Program

Vaccines for Children is a federally funded program to help provide important immunizations to children who might not otherwise be vaccinated because of inability to pay. To qualify a child must be 18 years of age or under and enrolled in Medicaid, uninsured, under-insured*, American Indian or Alaska native. A donation of $19.50 per immunization is requested. Immunizations offered include:







Varicella (Chickenpox)

DTaP

Hepatitis A

Hepatitis B

Hib

Influenza/Flu

Immunization clinics are open to the public and there are no income guidelines. A Licensed Nurse administers the vaccinations and a Physician is on-call during clinic hours.

Adult Immunizations

To help keep you and your family healthy, ELVPHD provides many vaccines for individuals of all ages including adults.   ELVPHD recognizes that you never outgrow your need for vaccines.   The specific immunizations you need as an adult depend on a variety of factors including you age, previous immunization history, high-risk conditions, and travel.   The majority of vaccines ELVPHD offers are covered by insurance as preventative medicine without requirement of a copay or co-insurance to meet your deductible. However, all individuals interested in receiving vaccines should check the immunization coverage allowed with their specific insurance plan.

 

Adult Vaccines Offered include:

  • Tdap (Tetanus, Diphtheria with Pertussis)

  • Hepatitis A vaccine

  • Hepatitis B vaccine

  • Pneumococcal vaccines (PCV13 & PPV23)

  • Zostavax (Shingles vaccine) – please check with your insurance provider on coverage for this vaccine.

  • Influenza or flu vaccine

  • High dose flu vaccine (age 65 and older)

     

    ELVPHD serves adult clients with both Private Insurance and Medicare and following is a list of insurance companies currently accepted:

  • Medicaid (United Healthcare, Total care & Well Care)

     Appointments are required for adults who intend to receive a vaccine other than influenza and Tdap to insure than we have the vaccine readily available.   Appointments can be made by calling 402-529-2233 between 8:00 a.m. to 4:30 p.m. Monday thru Friday.

Flu Vaccine

To help keep you and your family healthy, ELVPHD provides influenza vaccine for individuals of all ages.   Elkhorn Logan Valley Public Health Department is now serving both VFC and Privately Insured clients. To qualify for the VFC (vaccines for children) program a child must be at least 18 years of age or younger and meet one of the following: enrolled in Medicaid, be uninsured or have no health coverage, is American Indian or Alaskan Native, or be under-insured (has health insurance that does not cover the cost of vaccines).    In addition, ELVPHD serves clients with Private Insurance and/or Medicare for those age 65 and older.   The following is a list of insurance companies currently accepted:  

  • Blue Cross Blue Shield of Nebraska

  • Midland's Choice

  • United HealthCare

  • Coventry

  • Medicare

  • Medicaid (United HealthCare, Total care & Well Care)
